Issue Replacement or Change in Details of an Authorization/License/Curriculum

Service Description:

This service allows establishments and training centers to amend the data of an issued license or to obtain a replacement in the event of loss.

Requirements:

1. Official letter from the employer confirming the requested amendments or replacement.
2. Copy of the previous license (in case of amendment or replacement request).
3. A copy of a location map in case of changing of the location map

Service Fees

AED 300: Amendment of license data.

AED 200: Issuance of replacement for lost license.

AED 500: Site inspection fee (in case of new site/relocation).

AED 10: Innovation fee.

AED 10: Knowledge fee.

This service allows ambulance professionals to amend the data of their professional license or obtain a replacement in the event of loss.

Requirements:

1. Official letter from the employing establishment specifying the required amendments or requesting a replacement for a lost license.

AED 100: License data amendment / issuance of replacement for lost license.

AED 10: Innovation fee.

AED 10: Knowledge fee.

Service Delivery Steps

Service Description:

This service provides approval and accreditation of official training curricula offered by licensed training centres in the field of basic first aid.

Requirements:

1. Submission of a complete copy of the training curriculum (approved from the awarding body ) or need to be approved from DCAS.
2. Submission of supporting documents issued by the training center.
3. Compliance with the institution’s approved standards.

Service Fees

AED 200 for each training curriculum.

AED 10: Innovation fee.

AED 10: Knowledge fee.
